What to bring

A short checklist so check-in does not stall at the desk.

1. Photo ID
   Driver license or other government ID.
2. Insurance card
   We list sample payers on Location. Verify benefits yourself.
3. Medication list & allergies
   Names and doses beat “the blue one.”
4. Minors
   Parent or guardian must be present.
5. Payment
   Insurance and self-pay explained in plain numbers on a live site (demo: placeholder).
6. Labs tip
   Do not eat a huge meal right before if you might need labs — when unsure, call.
